You may use these forms for establishing and managing your Renaissance Charitable Gift Account. You have three options as to how you handle the forms.

• Print them out blank, fill them out, sign them and fax them to Renaissance at 877.222.1829.

• Fill out these forms on-line, print them out, sign them and fax them to Renaissance at 877.222.1829. 

• You may fill them out on-line, click on the "Submit" button at the bottom of the form to send them back via e-mail.

Note:  We recommend that you have Adobe Acrobat 6 or higher (or Acrobat Reader 7.0) installed to submit these forms on-line.  If you use Adobe Reader, you will not be able to save the forms with the content filled in, due to the limitations of this software, so be sure to use the "Print Form" button to generate a hard copy for your files before closing each form.  However, the on-line submit button will send the data to RCF.

If you are using an Internet-based e-mail program (such as Hotmail or Yahoo! you will be prompted to save the results file to your hard drive, then attach it to the e-mail when you open your mail program.  Please don't forget to attach this file before sending Renaissance Charitable Foundation the e-mail.  For Outlook or Eudora users, the results file will automatically attach to the e-mail when you click the e-mail button on the form.
