Advisors may download these forms for establishing and managing a Renaissance Charitable Gift Account for your clients. The Application, Program Circular, Change Form, Recommendation Form and Additional Contributions Form along with the "Owner's Manual", Investment Policy Statement and Advisor Instructions have been bundled into a self-extracting "zip" file* so you may download them to keep on your office PC for your use with clients.  The "Acknowledgement by Investment Manager" form that appears as Schedule A in the Investment Policy Statement is also part of the Zip file as a separate document so you may print it out separately and fax it to the Renaissance Charitable Foundation without having to print the entire Policy Statement.

You have two options as to how you handle the forms:

• Print them out blank, fill them out, sign them and fax them to Renaissance at 877.222.1829.
  
• Fill out these forms on-line, print them out, sign them and fax them to Renaissance at 877.222.1829.

*Note:  Because this file is self-extracting, it has an ".exe" extension that many computers running a virus scan will detect and warn you about downloading this file as it is a "program."  The file is a simple WinZip routine and will not install any additional programs onto your computer.  It will just place a folder onto your drive containing the Circular and forms.  If you have WinZip installed and wish to download the zipped file, click on the "non-self extracting" option instead.
