Instructions for Starting a Charitable Gift Annuity (CGA)

To establish a Charitable Gift Annuity, begin by completing a Renaissance Charitable Gift Annuity Application. The application requires information about the annuitant(s), donor(s), advisor, and the asset(s) being used to fund the annuity.

Mail the application along with a W-9 and a copy of the driver's license for each donor and annuitant to the Renaissance Charitable Foundation at: 6100 W. 96th Street, Suite 105, Indianapolis, IN 46278; or fax to (866) 736-4624.
